United Kingdom and Australia: Veterinary Group Reports a 200% Increase in Incident Reporting

CVS Veterinary Group says reports submitted through its VetSafe incident-reporting system increased from 761 to 2,275 over the preceding year—a rise of approximately 199%, rounded in the company’s announcement to 200%. The programme encourages clinical teams to record unexpected outcomes, conduct structured learning reviews and identify changes that could improve patient safety.

Examples of reported operational changes include revised surgical safety checklists, improved equipment-maintenance routines, changes to hospitalisation protocols and stronger medication-dispensing and patient-handover procedures.

An increase in reported incidents does not necessarily mean that the underlying incidence of clinical errors increased. It may reflect improved reporting, greater willingness to speak up, changes in system use, or a combination of factors. The company’s report links the increase to greater reporting and learning activity, but the figures alone cannot quantify the change in actual adverse-event rates.

Why it matters

Veterinary groups managing large networks can use structured incident reporting to identify recurring risks across sites. The wider lesson for veterinary businesses is that safety performance should be evaluated using both reporting activity and outcome measures, such as preventable harm, repeat incidents and compliance with corrective actions.
